Depending on the location of the protrusion, hernias receive the corresponding nomenclature. The most common sites for hernia formation are the inguinal canal (inguinal hernia), the umbilicus (umbilical hernia), the linea alba (epigastric hernia), and previous surgical incisions (incisional hernia). Less common hernias include femoral hernias, Spigelian hernias, and internal hernias.

Etiology: Hernias are classified as congenital or acquired, with the latter representing the vast majority. The main causes of hernia formation are increased intra-abdominal pressure and weakening of the abdominal wall. Therefore, conditions such as heavy and sudden lifting, paroxysmal coughing, chronic constipation, obesity, and prostatic hypertrophy can lead to hernia development. Predisposing factors also include lack of physical activity, natural aging, and the presence of surgical incisions.

Management: Once a hernia appears, the only definitive treatment is surgical repair. The goal of each operation is to reduce the hernia contents and close and reinforce the weakened point of the abdominal wall, usually with the use of a mesh. The decision regarding the timing and method of hernia repair must be made after a thorough analysis of the risks and benefits of each technique.

Potentially, all hernias have a small risk of developing what is known as incarceration, an emergency situation in which the protruding organ cannot return to the abdominal cavity. The patient experiences severe pain in the area, accompanied by swelling and possible redness. Hernia incarceration is always an urgent condition requiring immediate medical attention.

Obtaining a thorough medical and social history helps the surgeon propose the optimal solution for hernia management. Daily discomfort in the form of pain or swelling, impaired ability to work or exercise, are factors that should lead to hernia repair. Additionally, aesthetic concerns, particularly in large hernias, or disturbances in bowel or urinary function related to the hernia, are valid reasons for surgical intervention.

Surgical Techniques: The two main approaches are open and laparoscopic repair, each with its advantages and disadvantages, always based on mesh placement for hernia repair. The choice of technique should be individualized according to the patient’s history, including comorbidities and the presence of previous surgical scars. Current literature supports the use of laparoscopic techniques, which offer minimal postoperative pain, faster return to work, and comparable long-term recurrence rates.

ΚΗΛΗ

A hernia refers to the protrusion or prolapse of an intra-abdominal organ (usually intestine or omentum) through a weak point in the abdominal wall.
