It is caused by hair penetrating the skin, leading to an inflammatory reaction and the formation of an internal cyst. It occurs more commonly in young men, with predisposing factors including dense hair growth and a sedentary lifestyle. The main symptoms are redness in the area and pain, particularly during episodes of inflammation.<\/p><p><strong>Perianal abscess:<\/strong> This is a common condition that occurs around the anus due to the accumulation of pus in a small closed cavity. The primary cause is bacterial infection resulting from obstruction of the anal glands and crypts. Clinical presentation varies from mild tenderness and pus discharge to severe pain accompanied by high fever.<\/p><p><strong>Perianal fistula:<\/strong> This refers to the formation of an abnormal connection between the anal canal and the perianal skin. In 50% of cases, it develops as a result of a perianal abscess, but it may also be associated with inflammatory bowel disease. It usually presents with pus discharge from the anal canal or from the corresponding skin opening.<\/p><p><strong>Hemorrhoidal disease:<\/strong> This term refers to the symptoms and signs caused by abnormal function of the hemorrhoids. Hemorrhoids are vessels normally located in the distal part of the anal canal that contribute to maintaining normal blood flow and fecal continence. Their enlargement and resulting symptoms are caused by multiple factors, including chronic constipation, sedentary lifestyle, pregnancy, and heavy lifting. Symptoms may include swelling, bleeding, pain, and a range of other nonspecific complaints.<\/p><p><strong>Anal fissure:<\/strong> This is typically a superficial tear or wound at the end of the anal canal. Predisposing factors are similar to those for hemorrhoids, such as constipation. The main symptoms are severe pain, particularly during and after defecation, and bleeding.<\/p><p><strong>Management:<\/strong> Management of anal diseases can be divided into urgent and planned interventions.<br \/>Urgent conditions include drainage of a perianal abscess or an infected pilonidal cyst, as well as incision of a thrombosed hemorrhoid. These procedures are usually performed under local anesthesia in the private office and provide immediate symptom relief. After the acute phase subsides, definitive management is planned.<\/p><p>Definitive treatment requires a detailed assessment of symptoms, evaluation of the patient\u2019s needs, examination of the clinical picture, and potentially additional imaging studies. Management may be conservative or surgical. Conservative treatment can include dietary changes, medications, and guidance on local hygiene.<\/p><p>Surgical management should be individualized based on the size and location of the problem. It may involve traditional techniques, such as excision, or newer methods, including the use of ultrasonic devices or diode laser cauterization.
